Find StablePair hook addresses and live pool parameters on Ethereum and Sepolia.
The registered v4 hook is an ERC1967 proxy. Its address is permanent and encodes the hook permission flags; the implementation behind it changes on a governance upgrade. Always integrate against the hook address.
